Reviewing the service schedule, I found it calls for throttle cable replacement at 18K. I am wondering if that is really necessary. Can the more experienced among us provide a more realistic projection?

Are you sure it doesn't say adjustment? At 70k, I've never replaced mine.

In the Hanes service manual, Maintenance schedule, pg.1-3, Every 18,000 miles. Replace throttle cables (Section 24)
Has anyone had throttle cable failure?

I just ticked over 30k yesterday.
CHANGING the throttle cable has never been on my radar, nor will it be, until I sense an issue.
adjusting, along with the throttle cables, I have done yearly.

I took my 1y/o to me 04 1150r w/ 35k for a catch-up comprehensive service a couple of months ago. My mechanic twisted the throttle and recommended a new cable. I had one go on my 99GS w/40k. The whole works is really expensive and a pain in the ass to replace. There's a good u-tube video on it

There's your clue. Haynes are often referred to as "how not to do it".
