I agree with Celticus - my 2007R blew a piece off the top of the piston at 200kmh and my dealer replaced both connecting rods & bearings, both pistons & rings, both cylinders and one cylinder head plus the cat converter and also changed the oil & filter. Reason given for replacing the CAT was that the burnt / powdered ally and bits and pieces had gone in there and could cause problems later down the line.

Is there any concern of opinions on why a piston with <12K miles would fail?
Do these bikes generally run lean?
Certainly could be a random materials issue but then again it sounds as though the bike is stock, correct?
No aftermarket breathing or exhaust?

Not sure what a "concern of opinions" is - but am I concerned? Yes for the person who had the problem, no for the rest of us. The motor itself is well tried - same engine on earlier hexhead bikes, and it certainly doesn't have a reputation for blowing up. Just about the opposite.
I'd suspect a component flaw - not anything that will become endemic with the engines.
